  Do NOT use this form for: Medication Renewal 
            Medication renewals will no longer be processed over the internet.  To facilitate their renewal, please call your pharmacy and have your pharmacy forward a medication renewal form via a fax (706.769.8754) to our office.  This process helps eliminate errors on all fronts.  Please be sure to give us at least 3 working days.  If you need this sooner, have the pharmacist note this on the medication request and follow up with the pharmacist and/or our office in a reasonable amount of time.  With all of us looking out for one another, delays will hopefully be minimal.
